The Chlamydia and Gonorrhoea Test
This test is only for Chlamydia and Gonorrhoea, for a full sexual health screen visit a sexual health clinic (details on the back of this leaflet) or your GP

*INSIDE*:
Before the test
Fill out ALL of the yellow section of the form attached to the clear plastic sample bag.
Write your name, date of birth and when you did the test on one of the stickers on the side of the form and stick it on the small sample tube included in the pack.
If there is anything you are unsure of ask the doctor or a member of staff to help you.

The male test
Don't pass urine for at least 1 hour before your test
Try and make sure the your sample is taken as you begin to urinate.
Half fill the tube. Secure the lid.
Put the tube into the clear plastic bag attached to the form, seal the bag. Return the sample to the doctor, nurse or receptionist where you are taking the test.

The female test
Use tissue to wipe away any excess musus or vaginal excretions.
Remove the swab from the tube, insert into your vagina by about 3-4cm (1-1.5 inches) and turn the swab so it touches the inside walls of your vagina.
Carefully take it out and put it back inside the tube and push the lid down.
Put the tube into the clear plastic attached to the form, seal the bag.
Return the sample to the doctor, nurse or receptionist where you are taking the test.
